FC Royale Scholarship Application
Objective: FC Royale strives to keep soccer affordable and wishes to reduce the economic barriers of playing soccer. As such, as long as funds are available, FC Royale will make scholarships available to deserving players and their families.
FC Royale Scholarship Fund Overview:
- Scholarships will be granted on a yearly basis and must be applied for each year by the scholarship deadline.
- Scholarship families are requested to perform volunteer work for FC Royale. When scholarships are awarded, the responsible parent is asked to commit to volunteer hours (20 hours or 200 points for 49% or less support, 30 hours or 300 points for 50 – 74% support, and 40 hours or 400 points for 75% or higher support).
- Scholarships are applied to Club Registration fees only. Each scholarship family must pay for team fees.
Administration: This scholarship program will be under the direct supervision of the FC Royale Treasurer. The Board of Directors Scholarship Committee will make the determination of a family’s need for FC Royale.
Criteria and Eligibility: Any family is eligible to apply for a scholarship if an economic barrier would prevent the player from playing soccer. In order to assess financial need, the following will be taken into consideration in awarding the scholarship:
- Applicant must complete this FC Royale Scholarship Application Form below and submit the completed application along with a copy of the first two pages of the family's most recent federal tax return. You will receive an e-mail upon completion of this form detailing delivery options.
- Incomplete or late applications will not be considered.
Scholarship Awards: Application forms will be reviewed by the scholarship committee and coaching staff who will award scholarships based on financial need and team requirements.
Non-Discrimination Clause: No one will be disqualified from consideration because of sex, race, color, creed or religious beliefs.
Please provide a brief explanation of the need for financial aid, and whether are requesting a partial or maximum scholarship.
Waiver & Release of Liability
I understand that there are risks involved with my child’s participation in the FC Royale competitive soccer tryouts.
I hereby authorize the directors of FC Royale ("FC Royale") to act for me according to their reasonable judgment in any emergency requiring medical attention. I hereby waive and release the directors of FC Royale from all liability and agree to accept all medical expenses incurred. I know of no physical or mental problem that will affect my child’s ability to safely participate in these tryout sessions. I acknowledge and accept the conditions above with my signature below.
I certify that my child is in good health, and may participate in strenuous physical activities at the tryouts. I certify that there are no physical limitations to my child’s participation in the camp. Permission is granted for my child to receive emergency medical treatment if needed. I hereby release and forever discharge FC Royale and all their agents, employees and affiliated entities from any and all liability, claims, demands, and cause of action for personal injury or death, property damage, and/or other loss suffered by my child in connection with his/her participation in the tryouts. I acknowledge and accept that this Release and Waiver is intended to be binding on the family, estate, heirs, executors, administrators and assigns of the minor named above. I further acknowledge and accept that this Release and Waiver is intended to be as broad and inclusive as permitted by the laws of California and agree that if any portion of this release and Waiver is invalid, the remainder will continue to be in full force and effect. I agree that this Release and Waiver binds the minor and me to all of its terms.
I hereby grant permission to FC Royale and its legal representatives, assigns, and those acting on its behalf, to use any picture, video or audio recording of my child taken in connection with the tryouts for all manner of advertising, trade, promotion, exhibition, or any other lawful purpose related to youth soccer whatsoever and in any form or medium.
I hereby release the FC Royale and the California Youth Soccer Association – North, its member leagues, teams, agents, officers, coaches and players from all liability or responsibility for any claim, damage or legal action on behalf of the player or the player’s parents, heirs, or personal representatives, arising from any injury the player may sustain while participating in soccer or related activities, including transportation, except to the extent and in the amount covered by the CYSA-North accident reimbursement plan.

Return to Play per California Department of Public Health Guidelines of February 19th 2021
Informed Consent by Player and Family
The California Department of Public Health Guidelines published on 2/19/21 and beginning with
February 26, 2021 will allow for a return to play for youth sports under a variety of safety
measures and precautions. Improving factors in the pandemic including reduced case rates,
data demonstrating limited effects on youth, and increased availability of vaccinations, indicate
that a safe return to competition is possible as long as participants are aware of and honor
safety measures and protocols, care for their own exposure risks outside of sport activities and,
perhaps most importantly, care for at-risk persons in their daily paths who may be unprotected
by safety measures or vaccination.
It is therefore affirmed by the undersigned player and parent/guardian, that it is safe for said
player to return to play with FC Royale (“Club”) acknowledging the following
key tenants:
• I KNOWINGLY AND FREELY ASSUME ALL SUCH RISKS, both known and unknown, EVEN IF ARISING
FROM THE ACTIONS OF THE Club or others and assume full responsibility for my participation.
• The undersigned player is aware that a minor risk of virus exposure exists while participating in
youth sports, specifically soccer with the Club.
• The undersigned player is aware that the Club has no control over our opponent clubs and
teams (or any official or other organization) as to their adherence to state or local health
guidelines and enforcement.
• The undersigned player has reasonably assessed the virus exposure risk associated with
members of their household/immediate family and has taken and will take any and all
recommended steps to prevent such exposure, including, but not limited to, maintaining social
distancing, frequent washing of hands, the wearing of mask, and related items.
• The undersigned player will self-monitor and act appropriately by self-isolating from trainings
and/or games if symptoms are detected.
• The undersigned player will promptly notify the Club as to any known exposure.
Being fully aware of the factors outlined, I/We affirm our informed consent to participate in
soccer related activities with the De Anza Force and do hereby RELEASE, WAIVE, DISCHARGE,
AND COVENANT NOT TO SUE the Club, its Board of Directors, officers, employees, agents,
contractors, and representatives from any and all liability.
